Has your therapy provider assisted in addressing the prevention of pressure ulcers in your community? How well does your therapy provider communicate with nursing, the MDS Coordinator, and facility staff regarding changes in resident care and function?
Trinity Rehab Services is dedicated to developing true partnerships with open communication to ensure your residents achieve the highest level of independence. This includes involvement of the “whole” team, ours, and yours, in resident care including in the areas addressed by quality measures.
